Description
Shown from the distal femur to the foot, the male lower extremity is organized around the femur, patella, tibia, and fibula, with the tibia positioned medially and the fibula laterally, joined along their shafts by the interosseous membrane. Anterior to the tibia sit tibialis anterior with extensor hallucis longus and extensor digitorum longus, while the lateral compartment carries fibularis (peroneus) longus and brevis along the fibular shaft toward the lateral malleolus. Posteriorly, gastrocnemius overlies soleus, and deeper muscles (tibialis posterior, flexor digitorum longus, flexor hallucis longus) track inferiorly to the ankle, converging toward the calcaneus via the calcaneal (Achilles) tendon. Fascial planes partition the four compartments. Compartment anatomy is not academic here. Acute compartment syndrome most often affects the anterior compartment after tibial shaft fracture, where rising pressure threatens the deep fibular (peroneal) nerve and anterior tibial artery, producing pain with passive toe extension and early weakness of ankle dorsiflexion. The same compartment boundaries guide two-incision fasciotomy, and the relationship of the common fibular nerve to the fibular neck explains classic foot drop after trauma or prolonged compression at the lateral knee.
