Description

Posterior lower legs are presented in anatomical position, with the calf (crus) soft tissues opened to trace the principal arterial trunks running deep to the superficial fascia. The popliteal artery continues inferiorly to give the anterior tibial artery (passing through the proximal interosseous membrane toward the anterior compartment) and the tibioperoneal trunk, which divides into the posterior tibial artery and fibular (peroneal) artery descending along the posterior compartment. Distally, posterior tibial branches course toward the medial malleolus, while fibular perforators track laterally toward the distal fibula and lateral ankle. Small-caliber arterioles are suggested as terminal branching in the distal calf and hindfoot region. Clear laterality. Posterior mapping of these arteries matters because the posterior tibial artery and its accompanying tibial nerve sit in the tarsal tunnel behind the medial malleolus, a confined corridor relevant to hemorrhage control, decompression, and ultrasound-guided regional anesthesia. Proximal relationships in the popliteal fossa also anchor surgical planning, the popliteal artery lies deepest against the femur and posterior capsule, so knee dislocation and posterior tibial plateau trauma can threaten limb perfusion even when distal pulses seem equivocal. Short and unforgiving anatomy.
