Description
Emerging from the trigeminal ganglion, the maxillary nerve (V2) courses anteriorly as a purely sensory division of the trigeminal nerve before entering the pterygopalatine fossa. From an anterior perspective, V2 lies superior to the mandibular nerve (V3) and inferior to the ophthalmic nerve (V1), with the three divisions fanning from the lateral aspect of the brainstem and middle cranial fossa region. In the pterygopalatine fossa, V2 gives off ganglionic branches to the pterygopalatine ganglion and continues as the infraorbital nerve toward the orbit and midface. Spatial relationships matter here. Clinically, this depiction supports localization of facial sensory loss to the V2 territory, classically involving the lower eyelid, lateral nose, upper lip, maxillary teeth, and maxillary sinus mucosa. The segment of V2 in and around the foramen rotundum and pterygopalatine fossa is a common target for maxillary nerve block and radiofrequency procedures in refractory trigeminal neuralgia affecting V2, and it is also a corridor at risk during endoscopic sinus surgery or tumor spread in the pterygopalatine fossa. Seeing V2 in continuity with adjacent cranial nerves at the brainstem helps distinguish central lesions from peripheral entrapment or perineural invasion. Use this artwork in head and neck anatomy teaching blocks, dental anesthesia instruction, and neurology or neurosurgery materials discussing trigeminal neuropathic pain patterns and sensory mapping.
