Seen from a lateral cranial perspective, the large sensory root (portio major) of the trigeminal nerve (CN V) courses anteriorly from the lateral aspect of the pons toward Meckel’s cave, where it expands into the crescent-shaped trigeminal (semilunar, Gasserian) ganglion. From the ganglion’s anterior convexity, the three major divisions emerge in a fan-like arrangement: the ophthalmic nerve (V1) superiorly, the maxillary nerve (V2) anteromedially, and the mandibular nerve (V3) inferolaterally. The mandibular division typically appears thicker at its exit because it is joined by the smaller motor root (portio minor) that passes beneath the ganglion to merge with V3. Clear separation of root, ganglion, and divisions is the point. This lateral orientation matters when teaching the cisternal segment of CN V and its transition into the trigeminal cave, a key anatomic corridor for percutaneous procedures targeting trigeminal neuralgia. Needle trajectories aimed at the trigeminal ganglion via the foramen ovale risk preferentially affecting V3 and adjacent motor fibers, while compressive lesions near the ganglion can produce a division-specific sensory loss pattern across V1, V2, and V3 dermatomes.
