Description
Plantar to the tarsal and metatarsal skeleton, the plantar ligament complex spans from the calcaneus anteriorly toward the cuboid, navicular, cuneiforms, and bases of the metatarsals, forming a dense, layered buttress under the longitudinal and transverse arches. Distally, the phalanges of the hallux and lesser toes align anterior to the metatarsal heads, with the metatarsophalangeal joints marking the transition from midfoot support to forefoot propulsion. Medially, the first ray and hallux sit along the medial longitudinal arch, while the fourth and fifth rays angle laterally toward the cuboid and calcaneocuboid region. Clinically, the plantar calcaneonavicular ligament (spring ligament) and the long and short plantar ligaments are the quiet stabilizers of the midfoot, opposing talar head plantar-medial displacement and maintaining the calcaneocuboid and talonavicular joints during stance. Failure of this plantar support, often alongside posterior tibial tendon dysfunction, contributes to adult acquired flatfoot with collapse through the talonavicular joint and increased strain on the plantar fascia. A key teaching point. The relationship between these ligaments and the tarsometatarsal joints also frames discussions of Lisfranc injury patterns, where plantar ligament disruption predicts instability more reliably than dorsal findings alone.
