Description
Running deep to the external oblique and internal oblique, the transversus abdominis forms a broad muscular sheet wrapping the anterolateral abdominal wall, its fibers oriented predominantly transverse from the costal cartilages and lower ribs toward the linea alba. Superiorly it approaches the costal margin, while inferiorly it courses toward the iliac crest and inguinal region, blending into the thoracolumbar fascia posteriorly near the lumbar spine. Bony landmarks in this torso view logically include the thoracic cage with individual ribs, portions of the vertebral column posteriorly, and elements of the shoulder girdle (scapula and proximal humerus) at the superior lateral margin. Muscle layering is the point: superficial abdominal wall musculature frames the deeper plane. Transversus abdominis matters because it is the principal myofascial tensioner of the abdominal wall and a key contributor to circumferential trunk stiffness through its aponeurosis, linea alba, and connections to the thoracolumbar fascia. Surgeons and trainees often struggle to visualize where the transversus plane lies when discussing a transversus abdominis plane (TAP) block, and this layered anatomy also explains why lateral abdominal incisions can denervate segments and weaken the wall.
