Description
Running from the dorsal venous arch of the foot to the pelvic venous confluence, the superficial and deep venous systems are presented in a male lower body with the veins coded in blue. Medially, the great saphenous vein ascends anterior to the medial malleolus, tracks along the medial leg and thigh, and approaches the femoral vein at the saphenofemoral junction in the groin. Posteriorly and laterally, the small saphenous vein follows the midline of the calf toward the popliteal vein, while the deep trunks, including the anterior tibial, posterior tibial, and fibular (peroneal) veins, converge proximally into the popliteal and then the femoral vein, continuing as the external iliac vein superior to the inguinal region. Surface anatomy matters here. The long, uninterrupted course of the great saphenous vein, and its relationship to the medial knee and femoral triangle, makes it a common site for varicosities due to valvular incompetence and also a frequent conduit for coronary artery bypass grafting, with harvest planning guided by its position relative to the medial malleolus and the saphenous nerve. The small saphenous vein’s termination at the popliteal fossa helps explain why reflux patterns and perforator incompetence in the posterior calf can present differently from medial thigh disease. Faculty can drop this image into gross anatomy and vascular medicine teaching to contrast superficial tributaries with deep paired venae comitantes and to orient learners to clinical landmarks for venous access, duplex ultrasound reporting, and vein mapping before ablation or graft harvest.
