Description
Cut through the male kidney exposes the renal cortex peripherally and the medulla deep to it, with renal pyramids oriented toward the renal pelvis and minor and major calyces. At the renal hilum, the renal artery enters posterolateral to the renal vein in typical teaching arrangement, then divides into segmental and interlobar branches that course between pyramids before arching as arcuate vessels at the corticomedullary junction. Peripelvic and renal sinus fat occupies the central sinus around the calyces, pelvis, and proximal ureter, cushioning the vessels and collecting system as autonomic renal nerves track alongside the arterial tree. Small but dense anatomy. That central fat compartment matters clinically because it defines the renal sinus and changes how disease and surgery present: angiomyolipoma and lipomatosis can expand fatty tissue, while inflammatory stranding around the pelvis is a common CT sign of acute pyelonephritis or obstruction. For partial nephrectomy planning, the relationship of segmental arteries to the pyramids and calyces underpins selective clamping, limits ischemic injury, and helps you anticipate entry into the collecting system during tumor excision. Ureteropelvic junction obstruction also makes more sense when you can see how the funnel-shaped pelvis transitions into the ureter at the hilum.
