Description

Prominent at the medial border, the renal hilum contains the renal artery dividing into segmental branches and the renal vein converging anteriorly as it exits toward the inferior vena cava. Posterior to these vessels, the renal pelvis expands into major and minor calyces that extend laterally and superiorly toward the renal papillae, continuing inferiorly as the proximal ureter. The collecting system sits centrally, with the vascular tree coursing into the renal sinus alongside it. Nephrons are not individually resolved, but the calyceal arrangement implies the drainage territories of the renal pyramids. Clinical work around the renal hilum depends on clear vessel-pelvis-ureter relationships, because a small shift in dissection plane can trade a clean hilar control for an avoidable injury to the pelvis or a segmental artery. This view supports teaching of segmental renal arterial anatomy, the anatomic basis for partial nephrectomy and the end-artery concept, and it also helps explain why ureteropelvic junction obstruction produces hydronephrosis with calyceal ballooning upstream. Expect these landmarks to map directly onto CT urography and angiographic discussions, even though the presentation is model-like rather than radiologic.
