Description
Frontal operculum cortex occupies the inferior frontal gyrus along the superior lip of the lateral sulcus (Sylvian fissure), forming a cortical “lid” over the anterior insula. From a lateral cerebral hemisphere perspective, the opercular surface sits inferior to the middle frontal gyrus and anterior to the precentral gyrus, with the inferior frontal sulcus marking its superior boundary. Posteriorly, the region approaches the central sulcus via the precentral gyrus, while inferiorly it abuts the anterior temporal operculum across the fissure. Insular cortex lies deep to the operculum within the lateral sulcus. Small but crowded territory. Clinical relevance centers on language, swallowing, and operative corridors to the insula and middle cerebral artery (MCA) branches. The pars opercularis (Brodmann area 44) and adjacent pars triangularis (area 45) anchor the dominant-hemisphere Broca network, so infarction in the superior division of the MCA often produces nonfluent aphasia with apraxia of speech. Lesions involving the frontal operculum and adjacent insula can also contribute to opercular (Foix-Chavany-Marie) syndrome, with disproportionate impairment of voluntary orofacial and pharyngeal movements despite preserved reflexive function. For neurosurgeons, the fixed lateral relationship between opercular gyri and the Sylvian fissure helps orient subpial dissection and functional mapping when approaching insular gliomas or cavernous malformations, where cortical sparing is dictated by speech arrest and perirolandic motor responses.
