Description
Centered within the thorax, the heart sits in the middle mediastinum with its apex directed inferolaterally to the left and its base oriented posterosuperiorly. Both atria occupy the superior aspect of the cardiac silhouette, while the right and left ventricles form most of the anterior and inferior surfaces, separated by the anterior interventricular sulcus. At the superior surface, the ascending aorta and pulmonary trunk emerge anteriorly, and the superior vena cava enters the right atrium on the right; pulmonary veins are suggested posteriorly as they return to the left atrium. Surrounding thoracic contours and upper abdominal viscera, including loops of intestine inferior to the diaphragm, provide scale and anatomic context for an obese adult male body habitus. Obesity changes how clinicians think about the heart’s position and apparent size, both on physical exam and on chest radiography, where elevation of the diaphragm and increased mediastinal fat can broaden the cardiac silhouette without primary cardiomegaly. This framing supports teaching around obesity-associated cardiac remodeling, including left ventricular hypertrophy from hypertension, right-sided strain in obstructive sleep apnea with pulmonary hypertension, and epicardial adipose tissue as a marker linked to coronary artery disease.
