Description
Centered in the anterior neck, the hyoepiglottic ligament extends from the superior border of the epiglottic cartilage toward the body and greater horns of the hyoid bone, spanning the pre-epiglottic space in the midline. Superiorly, the epiglottis rises behind the tongue base, while inferiorly the laryngeal inlet leads toward the vestibular and vocal folds within the thyroid cartilage framework. Lateral to this ligament, the aryepiglottic folds sweep posterolaterally toward the arytenoid cartilages, and the cricoid cartilage forms a complete ring inferior to the thyroid cartilage. Small structure, big consequences. Otolaryngologists and anesthesiologists care about this tether because it influences epiglottic position during swallowing and during airway instrumentation. A lax or disrupted hyoepiglottic attachment can contribute to epiglottic prolapse and obstructive sleep apnea patterns, and the ligament is encountered when working through the vallecula during direct or video laryngoscopy. Its relationship to the pre-epiglottic fat and the thyrohyoid membrane also matters in supraglottic tumor spread and in planning supraglottic laryngectomy, where preserving functional suspension of the epiglottis and hyoid can affect postoperative swallowing. Use this illustration in head and neck anatomy teaching to orient learners to the hyoid, epiglottis, and intrinsic laryngeal muscles as a single functional unit, not isolated parts. It also fits well in airway management manuals, ENT surgical atlases, and patient-education materials explaining epiglottic collapse or supraglottic pathology.
