Description
Arising from the inner surface of the thyroid cartilage near the laryngeal vestibule, the thyroepiglottic muscle courses superiorly and posteriorly toward the lateral margin of the epiglottis, blending with fibers of the aryepiglottic fold. From a superior perspective you would expect to see the epiglottis forming the anterior boundary of the laryngeal inlet, with the paired aryepiglottic folds posterolaterally and the thyrohyoid membrane and thyroid cartilage lying inferiorly. Medial to the fold sits the aditus laryngis, while the muscle fibers sit more laterally as they fan toward the epiglottic cartilage and adjacent ligamentous tissue. Small but instructive. Functionally, the thyroepiglottic muscle is often taught as the counterpoint to the aryepiglottic muscle: it tends to widen the laryngeal inlet during respiration and phonation, resisting excessive inward collapse of the aryepiglottic folds. That relationship matters when interpreting supraglottic airway behavior in laryngomalacia, or when planning a supraglottoplasty where trimming of redundant aryepiglottic tissue can change the balance of forces around the epiglottis. The superior view also maps cleanly to what surgeons and laryngoscopists see during endoscopic evaluation, where orientation errors around the epiglottis and vestibular folds are common early in training.
