Description
Emerging from the lateral aspect of the ischial spine, the gemellus superior runs laterally and slightly anteriorly to blend with the tendon of obturator internus on its way to the medial surface of the greater trochanter. Superiorly it sits just inferior to piriformis and the greater sciatic foramen, while inferiorly it is separated from gemellus inferior by the obturator internus tendon. Medial to it lie the pelvic wall and ischium, and lateral to it the posterior aspect of the hip joint capsule and the posterior femur. A small muscle. A constant landmark. Functionally, gemellus superior contributes to external rotation of the thigh in extension and assists abduction when the hip is flexed, but its real teaching value is how it frames the obturator internus tendon within the deep posterior hip compartment. This relationship matters during posterior and posterolateral hip approaches and during evaluation of deep gluteal pain syndromes, where the sciatic nerve may be irritated as it courses inferior to piriformis and posterior to the short external rotators. Appreciating the ischial spine origin also helps orient learners to nearby neurovascular structures, including the pudendal nerve and internal pudendal vessels as they hook around the ischial spine near the sacrospinous ligament. Use this illustration in pelvic and gluteal region anatomy teaching, orthopaedic or sports medicine texts discussing short external rotators, and surgical education materials covering posterior hip exposure and repair of external rotator tendons.
