Description
Obturator internus and the short external rotators sit deep to gluteus maximus, and the gemellus superior is positioned immediately superior to the obturator internus tendon as it turns laterally out of the lesser sciatic foramen. From the pelvis, the muscle arises from the ischial spine and blends with the obturator internus tendon, running lateral and slightly anterior to insert onto the medial surface of the greater trochanter at the trochanteric fossa. Adjacent bony landmarks include the ilium and acetabulum superiorly, the ischium inferiorly, and the proximal femur laterally. Small, deep, and easy to miss. This angle matters because the gemellus superior functions as part of the triceps coxae (with obturator internus and gemellus inferior), stabilizing the femoral head in the acetabulum during gait while contributing to lateral rotation of the extended hip. Clinically, its relationship to the sciatic nerve as the nerve exits the greater sciatic foramen makes the region relevant in deep gluteal pain syndromes, where hypertrophy, spasm, or fibrosis of the obturator internus–gemelli complex can irritate the nerve and mimic lumbar radiculopathy. The ischial spine origin also helps orient learners to the nearby pudendal neurovascular bundle as it hooks around the spine toward the lesser sciatic foramen.
