Description

Deep in the posterior gluteal region, the gemellus superior appears as a short, triangular muscle spanning from the ischial spine to the tendon of obturator internus as it turns laterally toward the greater trochanter. Medial to it sits the sacrum and posterior ilium with the sacroiliac joints, while the proximal femur and acetabulum lie lateral and slightly inferior, defining the hip joint relationship. Inferiorly, the gemellus inferior and quadratus femoris would be expected in the same plane of the deep short external rotators, with the ischial tuberosity positioned more caudal and medial. Small muscle, big landmark. Posterior exposure matters here because the gemellus superior is best understood as part of the obturator internus complex, a unit that stabilizes the femoral head during hip extension and external rotation and becomes clinically relevant in deep gluteal pain patterns. This view supports discussion of deep gluteal syndrome, where the sciatic nerve can be irritated in the tight interval between the short external rotators and adjacent structures, and it also clarifies why posterior hip approaches (including posterior total hip arthroplasty) must respect the short rotators to reduce postoperative instability.
