Description
Positioned deep in the posterior hip, the gemellus inferior lies immediately inferior to the tendon of obturator internus and superior to quadratus femoris, forming a compact stack of short external rotators as they approach the medial aspect of the greater trochanter. From a posterior view in the male pelvis, the paired gemellus inferior muscles flank the obturator internus tendon laterally, while their medial attachments track toward the ischial tuberosity and ischial spine region. Fibers run laterally and slightly superiorly to blend with the common tendinous insertion shared by the obturator internus–gemelli complex. Small but exact. This angle matters because the gemellus inferior is best understood as part of a functional unit with obturator internus, a relationship that clarifies both leverage and the tight anatomic corridor adjacent to the sciatic nerve as it exits the greater sciatic foramen inferior to piriformis. In deep gluteal pain syndromes, fibrotic bands or hypertrophy in the obturator internus–gemelli region can irritate the sciatic nerve and mimic radiculopathy, so appreciating their posterior topography helps clinicians localize tenderness and plan targeted injections. The same anatomy guides posterior approaches to the hip, where the short external rotators are encountered and repaired to reduce postoperative instability.
