Description
Emerging deep in the posterior hip, the gemellus inferior lies immediately inferior to obturator internus and superior to quadratus femoris, spanning from the ischial tuberosity toward the medial aspect of the proximal femur. Its fibers blend with the obturator internus tendon as they course laterally to the trochanteric fossa on the posterior surface of the greater trochanter. From this posterior perspective, the pelvis and bilateral hip joints frame the muscle belly, with the sacrum and lower lumbar vertebrae positioned superiorly and the acetabula lateral to the pelvic midline. Close by are the ischial spine and lesser sciatic notch, landmarks that orient the short external rotators in the deep gluteal region. Small muscle. Big landmark. Clinically, this relationship matters during posterior approaches to the hip, where the conjoined tendon of obturator internus and the gemelli is often released and repaired to reduce postoperative instability.
