Description
Arising from the inner surface of the thyroid cartilage near the anterior commissure and continuing superiorly into the aryepiglottic fold, the thyroepiglottic muscle (thyroepiglotticus, a superior extension of the thyroarytenoid) is seen coursing toward the lateral margin of the epiglottis. Medial to it lies the laryngeal lumen with the vestibular fold superiorly and the true vocal fold inferiorly, while posteriorly the fibers relate to the arytenoid region within the framework formed by the thyroid and arytenoid cartilages. Superior is the epiglottis and pre-epiglottic space, inferior the subglottic airway leading to the cricoid level. Small fibers, big consequences. Functionally, this muscle helps modulate the laryngeal inlet by drawing the epiglottis and aryepiglottic fold anteroinferiorly, contributing to widening of the aditus laryngis during respiration and fine-tuning supraglottic configuration during phonation. That makes the lateral cross-sectional relationship clinically relevant when teaching laryngeal airway protection and when interpreting endoscopic findings in supraglottic edema, where thickening of the aryepiglottic fold can narrow the inlet and alter epiglottic posture. The same anatomy is encountered during supraglottoplasty for laryngomalacia and during transoral laser microsurgery of supraglottic lesions, where preserving the muscular sling can reduce postoperative aspiration risk. Use this view in head and neck anatomy curricula to anchor the continuity between the thyroarytenoid complex and the epiglottis, and in otolaryngology teaching files to support discussions of supraglottic obstruction, swallowing mechanics, and surgical corridors through the vallecula and aryepiglottic fold.
