Description
Arising from the anterior wall of the laryngopharynx, the epiglottic cartilage appears in lateral section as a leaf-like plate with its broad lamina directed superiorly and posteriorly toward the laryngeal inlet and its narrow petiolus anchored inferiorly to the thyroid cartilage at the level of the thyroepiglottic ligament. Immediately anterior to the free edge lies the vallecula between the epiglottis and the base of tongue, while posteriorly the epiglottis overhangs the aditus laryngis leading toward the vestibule and glottis. Surrounding laryngeal framework elements such as the thyroid cartilage and cricoid cartilage form the anterior and inferior boundaries of the airway in section, with adjacent intrinsic muscle bundles (including thyroarytenoid and cricothyroid fibers) oriented along the laryngeal walls. The thyrohyoid membrane and other fibrous connections are suggested where soft tissue bridges the cartilaginous margins. A lateral cross section is the clearest way to teach how the epiglottis moves as a passive elastic structure during swallowing while remaining integrated with the thyroid cartilage and pre-epiglottic soft tissues. Airway and anesthesia clinicians care about this geometry. The vallecula is the standard laryngoscopic landmark for Macintosh blade placement, and a high-riding or omega-shaped epiglottis can complicate direct laryngoscopy, while epiglottic edema in supraglottitis narrows the inlet at the same level shown here.
