Description
Running in the lateral column of the erector spinae, the iliocostalis thoracis is highlighted along the posterior thoracic wall, positioned lateral to longissimus thoracis and medial to the posterolateral rib cage. Superiorly, its slips approach the angles of the upper ribs and can be traced toward the transverse process of C7, while inferiorly it continues toward the angles of the lower ribs where it blends with the iliocostalis lumborum at the thoracolumbar fascia. Superficial layers such as the trapezius over the upper back and the lateral margin of latissimus dorsi are partially preserved, framing the deeper paraspinal muscle mass along the spinous processes. Clinically, isolating iliocostalis thoracis matters when teaching thoracic extension and ipsilateral lateral flexion, but also when explaining why posterior rib pain can accompany paraspinal strain after lifting, coughing, or prolonged flexed posture. Palpation and trigger point referral from the iliocostalis group can mimic costovertebral joint irritation, and the muscle often appears hypertrophied or asymmetric in chronic scoliosis or compensatory thoracic kyphosis. For posterior thoracic approaches, its relationship to the rib angles and the more medial longissimus helps orient dissection and retraction. Landmarks matter.
