Description
Running longitudinally along the posterolateral thoracic wall, the iliocostalis thoracis is highlighted as the lateral column of the erector spinae complex, positioned lateral to the spinous processes and medial to the posterior angles of the ribs. Its fascicles continue superiorly from the iliocostalis lumborum, arising in continuity from the iliac crest and thoracolumbar fascia region, then coursing cranially to attach on the inferior borders of multiple ribs. The thoracic and lumbar vertebrae form the midline reference, with the rib cage curving laterally and anteriorly away from the muscle belly, and the pelvis anchoring the caudal origin of the erector spinae mass. Posterior presentation is the clearest way to teach how the iliocostalis differs from the longissimus and spinalis by its rib-based insertions and its more lateral position, which becomes obvious once you track it from the iliac crest toward the lower ribs.
