Description
Arising from the ophthalmic division (V1) in the lateral wall of the cavernous sinus, the lacrimal nerve is traced anteriorly through the superior orbital fissure and into the orbit in a skull section. Along the superolateral orbit it courses on the superior surface of the lateral rectus muscle, positioned lateral to the optic nerve and superior to the main neurovascular bundle heading toward the apex. Distally it runs toward the lacrimal gland in the lacrimal fossa of the frontal bone, with small accompanying vessels in the periorbita. The globe sits centrally within the bony orbit, its sclera and blue iris oriented anteriorly. Clinically, the lacrimal nerve matters because it carries sensory fibers from the lacrimal gland, conjunctiva, and upper lateral eyelid, and it provides the route for postganglionic parasympathetic secretomotor fibers that join it from the zygomatic nerve (V2) after synapsing in the pterygopalatine ganglion. Injury along its superolateral course can present as hypoesthesia at the lateral upper lid or reduced reflex tearing, and the relationship to the lateral rectus makes it a relevant landmark during lateral orbitotomy or dissection toward the lacrimal gland.
