Description
Presented from the left lateral thoracic wall of an adult male, the rib cage and thoracic spine provide the bony backdrop for a highlighted lateral cutaneous branch emerging from an intercostal nerve. From the intercostal space, the nerve courses laterally between the ribs, then pierces the intercostal musculature and superficial fascia to reach the dermis and skin of the flank. Superiorly, the scapula overlays the upper ribs, while inferiorly the thoracic vertebrae transition toward the lumbar region with intervertebral discs visible in profile. Superficial sensory territory is implied by the branch’s subcutaneous trajectory. For teaching thoracic dermatomes, this lateral view is the money angle. It clarifies where the intercostal nerve’s lateral cutaneous branch becomes vulnerable at the midaxillary line, a common site of iatrogenic injury during chest tube insertion, thoracostomy, or lateral thoracic incisions, and a relevant pathway in intercostal neuralgia and post-thoracotomy pain syndromes. The relationship to the ribs and intercostal spaces also supports accurate placement of intercostal nerve blocks, where needle trajectory aims for the neurovascular plane along the inferior border of a rib while appreciating where cutaneous branches exit more laterally.
