Description
Emerging from the intercostal nerve within the thoracic wall, a lateral cutaneous branch is traced from the deep intercostal plane toward the superficial fascia and dermis of the anterolateral chest in an adult male. The rib cage is presented in anterior orientation with the sternum at the midline and ribs sweeping laterally, while thoracic vertebrae form the posterior axis seen through the bony framework. As it courses laterally, the sensory branch pierces the intercostal musculature near the midaxillary line and then divides toward anterior and posterior cutaneous territories over the skin. Sensory mapping of the thoracic wall depends on understanding where these lateral cutaneous branches exit, because they define the lateral limit of each intercostal dermatome and are commonly encountered when incisions or drains traverse the intercostal spaces. This is the anatomy behind post-thoracotomy pain syndrome and intercostal neuralgia, where injury or entrapment of the intercostal nerve or its lateral cutaneous branch produces bandlike chest wall dysesthesia that can mimic cardiopulmonary pain.
