Description
Arising from the olfactory bulbs on the inferior surface of the frontal lobes, multiple olfactory nerve filaments (fila olfactoria, CN I) descend to the cribriform plate of the ethmoid, where they traverse its foramina to reach the olfactory mucosa of the superior nasal cavity. Superior to the cribriform plate, the olfactory tract runs posteriorly toward the olfactory trigone, positioned anteromedial to the anterior perforated substance. Fine arterial branches course alongside the ventral forebrain and brainstem, rendered in blue, while the yellow cranial nerves radiate laterally and inferiorly from the brainstem and basal surface of the brain in the expected topography. Clinically, the cribriform plate is a frequent point of failure in anterior skull base trauma, and shearing of the fila olfactoria as they pass through the ethmoid can produce anosmia even when the bulbs and tracts appear intact on gross inspection. A thin, porous cribriform plate also explains why a cerebrospinal fluid rhinorrhea may accompany loss of smell, creating a direct route between the nasal cavity and anterior cranial fossa with meningitis risk. Small anatomy, big consequences.
