Description
Obliquely from the posterolateral knee, the popliteus appears as a triangular muscle arising from the lateral femoral condyle and sweeping inferomedially to the posterior surface of the proximal tibia, coursing deep to gastrocnemius and superficial to the posterior tibial cortex. Lateral to its tibial attachment, the fibular head and proximal fibula anchor the fibularis (peroneus) longus and brevis more distally along the lateral compartment, while the tibia remains medial and dominant as it continues toward the ankle mortise. Distally, the segmented tarsals, metatarsals, and phalanges are rendered in alignment with the tibia and fibula, reinforcing longitudinal relationships from knee to foot. Small muscle. Big landmark. Clinically, popliteus matters because it initiates knee flexion by “unlocking” the joint, rotating the tibia medially on the femur in open-chain motion (or rotating the femur laterally on a fixed tibia), a concept that is hard to teach without a clear view of its oblique fiber direction across the posterolateral capsule. The muscle’s proximity to the lateral meniscus and posterolateral corner structures makes it a recurring reference point when discussing posterolateral rotatory instability, PCL-associated injuries, and surgical approaches that work between biceps femoris, the fibular head, and the lateral gastrocnemius while protecting the common fibular nerve.
