Description
Seen from a posterior perspective in an adult male, the distal femur, proximal tibia, and proximal fibula frame the knee while the talus sits inferiorly at the ankle mortise formed by the distal tibia and fibula. The popliteus muscle occupies the deep posterior knee, running obliquely from the lateral femoral condyle superolaterally to its insertion on the posterior tibial surface superomedial to the soleal line. Articular cartilage is rendered on the femorotibial surfaces, clarifying joint congruence behind the popliteal fossa. Deep and angled. Popliteus anatomy matters most when you are teaching or treating posterolateral corner mechanics: its tendon and muscle belly counter external rotation of the tibia and help initiate knee flexion from terminal extension, the classic “unlocking” action that becomes clinically obvious in gait after injury. Because the popliteus tendon lies adjacent to the lateral meniscus and courses near the lateral femoral condyle, tendinopathy, avulsion, or musculotendinous strain can mimic lateral meniscal pathology, and it is a named structure to scrutinize on knee MRI in sports injuries. Orthopedic approaches to the posterolateral knee also use the popliteus as a landmark when working around the fibular head region where the common fibular (peroneal) nerve is at risk.
