Description
Positioned deep in the posterior knee, the popliteus appears as a slender, flat muscle exposed from a medial orientation within the popliteal fossa. Its tendon courses proximolaterally toward the lateral femoral condyle and the posterior horn region of the lateral meniscus, then the muscle belly runs inferomedially across the posterior tibia, lying anterior to the medial head of gastrocnemius and adjacent to the posterior joint capsule. Medially, the tibial condyle and proximal shaft provide the bony backdrop, while the femur remains superior and the tibia and fibula continue distally toward the ankle and foot skeleton. Medial viewing angles clarify why popliteus is taught as the key “unlocker” of the knee: it internally rotates the tibia on the femur in open-chain movement, or externally rotates the femur on a fixed tibia during stance, initiating flexion from full extension. That relationship matters in posterolateral corner assessments and in knee arthroscopy, where the popliteus tendon and popliteal hiatus form a landmark near the lateral meniscus and help orient surgeons working close to the posterior capsule.
