Description
Positioned deep in the posterior knee, the popliteus appears as a flat, triangular muscle forming the floor of the popliteal fossa, with its broad distal fibers spreading inferiorly and medially across the posterior surface of the proximal tibia. Superficial to it sit the medial and lateral heads of gastrocnemius, while the biceps femoris tendon tracks down the posterolateral thigh toward the fibular head and the hamstring mass lies superior to the fossa. Lateral relationships include the iliotibial tract and vastus lateralis at the thigh, while inferiorly the soleus and the calcaneal (Achilles) tendon dominate the posterior compartment contour. Deep. Compact. Easy to miss. Clinically, this view matters because the popliteus is the key “unlatching” muscle of the knee, internally rotating the tibia on the femur in open-chain motion (or externally rotating the femur on a fixed tibia) to initiate flexion from terminal extension. Its tendon courses deep to the lateral collateral ligament and along the lateral femoral condyle, so posterolateral corner injuries and lateral meniscal pathology are often discussed alongside popliteus strain or tendon instability. Surgeons also use the popliteus and the surrounding fat and fascia planes as landmarks when orienting within the popliteal fossa, where the tibial nerve and popliteal vessels run superficially to the muscle. Use this posterior knee depiction to teach the layered anatomy of the popliteal fossa in gross anatomy and kinesiology courses, or to support orthopedic and sports medicine content on posterolateral corner reconstruction, meniscal repair, and differential diagnosis of posterior knee pain.
