Description
Ventral pallidum occupies the basal forebrain in this anterior brain section, positioned inferior to the anterior commissure and continuous laterally with the pallidal territory deep to the ventral striatum. Medially, its gray matter approaches the septal region and substantia innominata, while laterally it abuts white matter bundles that separate it from the putamen and adjacent striatal tissue. Superior to the ventral pallidum, the section typically captures parts of the head of the caudate nucleus and anterior limb of the internal capsule, framing the relationship between ventral pallidum and the ventral striatum (including nucleus accumbens) in a single fixed plane. Boundaries are defined by the commissural fibers above and the pallidostriatal transition zone laterally. Clinically, the ventral pallidum is the principal pallidal output of the limbic basal ganglia loop, receiving dense input from the nucleus accumbens and projecting onward toward mediodorsal thalamic and brainstem targets, which is why pathology here maps so cleanly onto reward processing, motivation, and anhedonia. A focal infarct or hemorrhage in the basal forebrain can disrupt this circuitry and present with apathy, reduced goal-directed behavior, or abnormal reward valuation, and the nearby anterior commissure provides a consistent landmark when orienting coronal sections in neuroanatomy teaching and surgical planning. Orientation matters. The anterior placement emphasizes the ventral striatal-pallidal interface rather than the classic dorsal globus pallidus seen in more posterior sections.
