Description
Basal forebrain circuitry comes into focus around the sublenticular extended amygdala (SLEA), a continuous cell band positioned inferior to the lentiform nucleus in an anterior section of the human brain. In this slice, the putamen and globus pallidus sit dorsolaterally, while the SLEA occupies the sublenticular region medial to the insular cortex and lateral to the hypothalamic and septal territories. Fibrous boundaries expected at this level include the anterior limb and genu of the internal capsule superiorly and the sublenticular white matter planes that course toward the temporal lobe inferiorly, framing the SLEA as it bridges limbic and striatal neighborhoods. Continuity with the extended amygdala complex is implied along the ventral basal forebrain, linking amygdaloid and bed nucleus of the stria terminalis regions across the anterior forebrain. Clinically, the SLEA is a useful concept when teaching how “amygdala-related” functions extend beyond the temporal lobe proper, because its cell groups track beneath the basal ganglia where lesions, hemorrhage, tumor mass effect, or surgical corridors can interrupt limbic output pathways without a primary temporal lobe abnormality. This anatomical relationship helps explain why patients with ventral basal forebrain injury may present with altered fear conditioning, stress reactivity, or motivated behavior, and why neuromodulation targets in neighboring ventral striatal and basal forebrain zones demand careful respect for sublenticular fiber systems. The fixed anterior sectional perspective supports precise localization relative to the lentiform nucleus and internal capsule, a common point of confusion in trainees.
