Description
Zygomaticoorbital foramina punctuate the orbital surface of the zygomatic bone along the anterolateral wall of the bony orbit. From an anterior orbital perspective, the openings sit on the smooth concavity of the zygomatic bone just deep to the lateral orbital rim, medial to the zygomaticofrontal region and superior to the zygomaticomaxillary area. Their position places them lateral to the orbital cavity proper and proximal to the course of the zygomatic nerve as it enters the orbit from the pterygopalatine fossa via the inferior orbital fissure. Several foramina may be present, clustered rather than solitary. Small openings, big sensory consequences. These foramina transmit orbital branches of the zygomatic nerve (V2), which subdivide into zygomaticofacial and zygomaticotemporal nerves supplying sensation over the malar eminence and anterior temple, and their caliber and number vary enough that surface landmarks alone can mislead during local anesthetic blocks or surgical dissection. In orbitozygomatic fractures and zygomaticomaxillary complex injuries, fracture lines can traverse the thin orbital surface, and awareness of the foramina helps explain postoperative dysesthesia or neuropathic pain even when the infraorbital foramen is uninvolved. The isolated bony view also supports teaching the distinction between zygomaticoorbital foramina on the orbital surface and the zygomaticofacial foramen on the lateral facial surface, a common point of confusion in gross anatomy and radiology correlation.
