Running along the anterior aspect of the interosseous membrane, the paired anterior interosseous veins ascend in the deep volar forearm alongside the anterior interosseous artery, positioned between the radius laterally and the ulna medially. Proximally, these venae comitantes track toward the cubital fossa to join the deep venous network that converges on the brachial veins, while distally they receive tributaries from the wrist and deep palmar venous channels near the carpal bones. Superficial return is also present, with the cephalic vein on the lateral forearm and arm and the basilic vein medially, both connected by perforators to the deep system around the elbow. Bony landmarks are kept in view. Anterior interosseous venous anatomy matters most when you are trying to keep deep and superficial pathways straight during vascular access planning and when interpreting forearm swelling in the setting of trauma or infection. Deep venous thrombosis of the upper limb more often involves the brachial, axillary, or subclavian segments, but thrombus or catheter-related occlusion can propagate from the paired deep forearm veins, and the close companionship of vein and artery helps explain why deep collections in the flexor compartment can compromise both venous outflow and arterial inflow. Surgeons also use these relationships during volar approaches to the radius and ulna, where bleeding may track along the interosseous membrane and obscure the field.
