Rendered from a lateral perspective, the anterior lymphatics of the prostate are traced along the anterolateral surface of the gland, immediately inferior to the urinary bladder and anterior to the rectum. Fine lymphatic vessels course superiorly toward the pelvic sidewall, running in close company with the prostatic venous plexus and branches of the internal iliac vessels, while the pelvic floor musculature frames the inferior aspect of the prostate near the urogenital diaphragm. Portions of the pelvic ring provide bony orientation, and adjacent bowel loops sit more superiorly and posteriorly in the field. Anterior prostatic drainage matters because it parallels routes of nodal spread in prostate carcinoma, with first-echelon involvement commonly seen in obturator and internal iliac nodal basins and potential extension to external iliac nodes depending on tumor location and burden. A lateral depiction also clarifies why pelvic lymph node dissection and nerve-sparing radical prostatectomy demand constant attention to the proximity of lymphatic channels, the prostatic plexus, and the pelvic autonomic nerves near the lateral prostatic fascia.
