Anterior pelvic anatomy is presented with the urinary bladder superiorly and the prostate gland immediately inferior to the bladder neck, framing the proximal urethra as it descends toward the membranous urethra. Fine lymphatic vessels course over the anterior prostatic capsule, then track superolaterally toward the periprostatic and internal iliac nodal basins, with smaller channels converging near the prostatic utricle in the midline. Ureters enter the bladder at the superolateral angles, providing orientation for the trigone and the anterior bladder wall relative to the gland. Midline matters here. Understanding anterior prostate lymphatics is central to how prostatic carcinoma spreads and how pelvic nodal staging is planned, because capsular and pericapsular lymphatic networks can drain to obturator and internal iliac nodes even when the primary lesion sits anteriorly and is less conspicuous on digital rectal exam. This view aligns with the surgical field encountered during retropubic prostatectomy, where control of the dorsal venous complex and dissection along the anterior capsule place these lymphatic channels at risk for disruption. It also supports interpretation of sentinel node techniques and PSMA PET correlates when nodal disease appears in expected pelvic chains.
