Description
Posterior pelvic anatomy centers on the urinary bladder superiorly and the prostate gland immediately inferior to the bladder neck, with the ureters entering the posterolateral bladder wall and coursing obliquely before reaching the trigone. Along the dorsum of the prostate, laterodorsal lymphatic vessels track inferomedially from the posterolateral prostatic capsule toward the periprostatic plexus and then posterolaterally toward the internal iliac (hypogastric) chain. Neurovascular elements accompany this route, with pelvic autonomic nerves and vessels running along the posterolateral surface of the gland near the prostatic pedicles. Spatial relationships are clear. Laterodorsal prostatic lymph drainage matters because posterior and posterolateral pathways form a common route for nodal spread in prostate carcinoma, often reaching obturator and internal iliac nodal basins before more proximal pelvic stations. Surgeons planning nerve-sparing radical prostatectomy and pelvic lymph node dissection use these relationships to anticipate where lymphatics and accompanying vessels cluster near the capsule, a region where traction, thermal injury, or bleeding can compromise the neurovascular bundles and continence-related support. This posterior perspective also clarifies why posterior capsular invasion correlates with nodal involvement on staging.
