Description

Posteriorly, the male pelvis and proximal lower limbs are arranged around the sacrum and hip joints, with the femora descending to the knees. Deep lymphatic vessels course within the posterior thigh, running in close company with the femoral vascular bundle in the proximal limb and then tracking toward the popliteal fossa distally. Expect deep inguinal lymph nodes superiorly (medial to the femoral vein) and popliteal lymph nodes posterior to the knee, with collecting vessels lying deep to the hamstring compartment and along the adductor canal region as they ascend. Correlation with the deep venous system is the point of this view. Deep lymphatics of the thigh follow the femoral and popliteal vessels, which is why lymphangitis or malignant spread from deep tissues can present with tender deep inguinal nodes even when superficial inguinal nodes are quiet. This posterior perspective also suits teaching the popliteal fossa as a crowded crossroads, where popliteal lymph nodes sit near the popliteal artery and vein and can be encountered during vascular exposure or evaluation of a posterior knee mass.
