Description
Against the anterior skull and facial skeleton, buccal lymphatic vessels course superficially across the cheek, tracking lateral to the nasal aperture and inferior to the orbit before descending toward the upper cervical region. Small buccal (facial) lymph nodes cluster along the course of these vessels overlying the maxilla and mandible, with efferents passing inferomedially toward the submandibular nodes along the inferior border of the mandibular body. Auricular anatomy remains visible laterally, helping orient the drainage pathways toward the parotid region, even though the facial musculature has been removed to clear the field. Color coding separates lymphatic vessels (blue) from nodal stations (green). Stripping away the muscles is a practical choice in the cheek, where the buccinator, zygomaticus major and minor, and masseter can otherwise obscure the relationship between superficial facial lymphatics and the bony landmarks used in surface anatomy. This is the drainage map you need when teaching why infections and malignancies of the upper lip, buccal mucosa, and lateral nose commonly present with submandibular and facial nodal enlargement, and why odontogenic infection from maxillary teeth can track to the same basins.
