Description
Centered on the midface in an anterior orientation, the buccal lymphatics appear as fine superficial vessels coursing within the cheek over the buccinator muscle and adjacent to the orbicularis oris, set against the layered musculature of the face. Small buccal lymph nodes are suggested along these channels, positioned lateral to the oral commissure and inferior to the zygomatic region, with drainage trending posteriorly toward the anterior border of the masseter. Superiorly, lymphatic channels relate to the infraorbital region beneath orbicularis oculi, while inferiorly they approach the mandibular body and the upper cervical field where platysma becomes visible. Clinically, buccal drainage matters because infections and malignancies of the cheek and lateral upper lip may bypass the submental chain and instead track toward facial and parotid nodes, then onward to the superficial and deep cervical lymph nodes along the internal jugular vein. This view supports teaching of why odontogenic infection from maxillary premolars and adjacent buccal gingiva can present with cheek swelling and tender facial nodes, and why incisions placed too close to the facial vein risk postoperative lymphatic leak and edema. The surrounding facial musculature provides surgical landmarks for procedures that traverse the buccal space, including intraoral drainage and approaches near the buccal fat pad. Faculty can drop this plate into head and neck anatomy labs when introducing superficial lymphatic territories and their relationship to mimetic muscles, or into dental and ENT teaching files covering patterns of lymphatic spread from the oral cavity and midface. It also fits well in patient education materials explaining why a cheek lesion may prompt neck palpation and targeted ultrasound of regional nodal basins.
