Description
Running deep to the sternocleidomastoid, the upper deep cervical lymphatics track along the internal jugular vein within the carotid sheath, with clustered deep cervical lymph nodes positioned inferior to the angle of the mandible and superior to the level of the omohyoid. Medial to this nodal chain lie the common and internal carotid arteries, while the vagus nerve descends posteriorly between artery and vein. Posterior and slightly lateral, the cervical plexus emerges from the ventral rami and courses beneath the prevertebral fascia toward the posterior triangle. Spatially, these lymphatic vessels and nodes sit deep to the investing layer of deep cervical fascia and anterior to the prevertebral musculature. A sweeping oblique angle through the upper neck clarifies why the jugulodigastric (tonsillar) region remains a first-pass drainage basin for the palatine tonsil, posterior tongue, and adjacent pharyngeal mucosa, and why these nodes enlarge early in acute tonsillitis and oropharyngeal carcinoma. Surgical dissection in levels II and III demands constant awareness of the carotid sheath contents and the spinal accessory nerve as it crosses the upper internal jugular region toward the sternocleidomastoid. Palpation landmarks matter. This anatomy determines what you feel, and what you should not injure.
