Description
Oriented from an inferior perspective beneath the male pelvic outlet, the prostatic apex sits superior to the urogenital diaphragm, with laterodorsal lymphatic vessels tracking along the posterolateral surface of the gland toward the pararectal and internal iliac chain. Fine lymph channels course proximally from the peripheral zone, running lateral to the prostatic urethra and posterior to the pubic symphysis, with the neurovascular bundle region implied along the dorsolateral capsule. Medially, drainage converges near the midline posterior surface adjacent to the rectum, while more lateral collectors follow the pelvic fascia toward obturator and hypogastric nodal basins. Small-caliber vessels dominate. Spatially, the pathway is inferior to the bladder base and seminal vesicles, yet posterior to the membranous urethra. Laterodorsal prostate lymphatics matter because they describe a common route for nodal metastasis in clinically localized prostate adenocarcinoma, often reaching internal iliac and obturator nodes before more cranial common iliac stations. This angle also maps closely to where surgeons work during radical prostatectomy and extended pelvic lymph node dissection, when preservation of the dorsolateral neurovascular bundle competes with oncologic clearance. Teaching this pathway from below clarifies why apical tumors can seed nodes even when the gland appears organ-confined on MRI.
