Description

Centered in the anterior abdominal cavity, the jejunum and ileum fill the periumbilical region as convoluted loops suspended from the posterior abdominal wall by the mesentery proper. Fine intestinal lymphatic vessels (lacteals within the villi continuing as mesenteric collecting lymphatics) track alongside the jejunal and ileal branches of the superior mesenteric artery and vein, converging proximally toward mesenteric lymph nodes. Superiorly and to the patient’s right, the duodenojejunal junction and proximal jejunal mesentery lead the eye toward the root of the mesentery, where lymph channels pass posterior to the peritoneal plane toward preaortic nodes and the cisterna chyli. Nerve filaments consistent with the superior mesenteric plexus may be suggested around the mesenteric vessels. Clear orientation matters. Functionally, this is the fat-transport pathway of the gut, carrying chylomicron-rich lymph from the jejunal and ileal mucosa to the thoracic duct rather than directly into the portal venous system. Obstruction or disruption of these channels explains postoperative or traumatic chylous ascites, and mesenteric lymphoma often presents with nodal enlargement clustered along the superior mesenteric vessels. The anterior perspective also supports operative thinking for small-bowel resection and mesenteric lymphadenectomy, where preserving arterial arcades and recognizing nodal basins at the mesenteric root reduces ischemic risk.
