Description
Anteriorly oriented, the male urinary bladder (vesica urinaria) is rendered with the detrusor muscle texture visible across the superior surface, body, and fundus as it tapers inferiorly into the bladder neck and proximal urethra. Lymphatic vessels course over the vesical wall and converge toward regional lymph nodes along the pelvic sidewall, with pathways expected to track laterally with the superior and inferior vesical vessels and posteriorly toward the internal iliac chain. Paired ureters enter the bladder superoposteriorly, their distal segments approaching the trigonal region deep to the mucosa, while the urethra exits inferiorly in the midline. Autonomic nerve pathways are included, aligning with the vesical plexus distribution around the base of the bladder. A dense neurovascular neighborhood. This anterior depiction matters because bladder lymphatic drainage is neither uniform nor clinically forgiving: lesions at the bladder base and neck tend to drain to internal iliac and sacral nodes, while the superior bladder more often routes toward external iliac nodes, patterns that influence staging and nodal dissection templates in muscle-invasive urothelial carcinoma. It also clarifies why anterior wall tumors can present with nodal disease along the obturator region even when the mucosal extent appears limited on cystoscopy, and why pelvic irradiation fields are planned with specific nodal basins in mind.
