Description
Centered in a posterior view, the urinary bladder (vesica urinaria) is shown with the trigone and bladder base oriented inferiorly toward the prostate, where the prostatic urethra begins in the midline. Ureters enter the posterolateral bladder wall obliquely at the ureterovesical junctions, framing the superior corners of the trigone. A green lymphatic network invests the posterior bladder and tracks inferolaterally toward the expected routes to perivesical nodes and onward to internal iliac and sacral nodal groups, closely paralleling the neurovascular planes around the bladder base. The prostate sits immediately inferior to the bladder neck, forming a continuous channel into the urethra. Posterior bladder lymphatics matter because they are the drainage highway for tumors arising from the trigone and bladder neck, areas with early access to pelvic lymph node basins. Surgeons planning radical cystectomy with pelvic lymph node dissection, or staging node-positive bladder cancer, think in precisely these pathways: perivesical to obturator, internal iliac (hypogastric), and presacral nodes, with crossover at the base that can confound laterality.
