Description
Viewed from an inferior aspect, the male urinary bladder (vesica urinaria) is presented with its lymphatic vessels coursing along the bladder base and inferolateral surfaces toward regional lymph nodes. Fine collecting trunks track alongside the vesical arteries and veins, converging posteriorly near the bladder neck and the proximal urethra, where lymphatic channels often form short plexiform connections before turning laterally. Medially, drainage relates to the midline trigone, while more lateral territories follow the contour of the detrusor toward pelvic sidewall nodal basins. Orientation is unambiguous. This perspective matters because bladder lymphatic outflow is not symmetric in clinical disease, and tumor location influences the first-echelon nodes at risk in urothelial carcinoma staging. Lesions at the bladder base and trigone commonly drain to internal iliac and obturator nodes, while more superior (dome) territories more often reach external iliac nodes; posterior-inferior pathways can involve presacral and common iliac chains in advanced spread.
