Description

Seen in left lateral profile, the left main bronchus enters the hilum and continues as the left lobar (secondary) bronchus to the superior lobe, giving rise to the anterior segmental (tertiary) bronchus. This anterior segmental bronchus courses anterolaterally within the upper lobe parenchyma, positioned anterior to the more posterior segmental branches and superior to the lingular bronchus. Segmental branching is presented as part of the bronchial arborization, with the tertiary airway narrowing as it runs distally toward its segment. Segmental anatomy matters because bronchopulmonary segments behave like surgical units, each supplied by its own segmental bronchus and branch of the pulmonary artery, while pulmonary veins run in the intersegmental planes. Clear identification of the left upper lobe anterior segment supports planning for segmentectomy and for bronchoscopic navigation when sampling a localized lesion confined to the anterior segment, a common scenario in early-stage peripheral non small cell lung cancer. It also helps explain why postobstructive atelectasis or pneumonia can remain segment limited when a segmental bronchus is occluded by mucus plugging or an endobronchial mass.
