Positioned along the anterior surface of the pancreatic head, the anterior superior pancreaticoduodenal vein courses within the pancreaticoduodenal groove adjacent to the second part of the duodenum. Superiorly it ascends toward the pyloroduodenal region, where it typically converges with other pancreaticoduodenal tributaries and drains into the portal venous system via the superior mesenteric vein or directly near the portal confluence. Surrounding landmarks in this anterior male view include the liver in the right upper quadrant, the stomach and transverse colon lying anterior to the pancreas, and the spleen laterally on the left. Fine venous branches from the proximal duodenum and superior aspect of the pancreatic head feed into this vessel.
