Description

Running from the anterior margin of the medial malleolus of the tibia to the anteromedial surface of the talus, the anterior tibiotalar ligament forms the most anterior portion of the deltoid ligament complex. A medial ankle view also brings the tibial plafond and trochlea tali into alignment, with the ligament blending anteroinferiorly into the joint capsule (capsula articularis) and the medial funiculus-like fiber bundle that tightens in dorsiflexion. Posterior and inferior to the highlighted band, the calcaneus and sustentaculum tali sit beneath the talus, while the navicular lies distal to the talar head. Tendons crossing this region, when included, sit superficial to the ligament, with tibialis posterior and flexor digitorum longus passing posteromedial to the malleolus. Medial ankle sprains are less common than lateral injuries, but when eversion and external rotation force the talus against the medial malleolus, the anterior tibiotalar fibers often take the first load, sometimes in association with syndesmotic injury or a medial malleolar avulsion. This view supports teaching of the talar tilt and anterior drawer concepts from the medial side, where instability can be subtle and is frequently described in reports as deltoid insufficiency rather than a single-band tear.
