Description

Centered in the right lower quadrant, the vermiform appendix extends from the posteromedial wall of the cecum, just inferior to the ileocecal junction, while the appendicular vein tracks proximally within the mesoappendix toward the ileocolic venous system. A posterior perspective keeps the cecum and ascending colon in the near field, with the distal ileum and coiled jejunoileal loops occupying a more medial position. Blue venous channels course in parallel with the appendicular artery, and the larger mesenteric vessels ascend superiorly toward the portal venous inflow near the liver. Orientation is clear. For appendectomy planning, the mesoappendix is where bleeding declares itself, and the appendicular vein is often the structure you meet first when dividing the mesoappendix close to the appendix to avoid injury to ileocolic branches. This view also helps explain how appendicitis can seed pylephlebitis, with septic thrombosis propagating from small veins in the mesoappendix into the ileocolic vein and onward to the portal venous system, a pathway that matters when right lower quadrant infection accompanies fever, abnormal liver enzymes, or hepatic abscess. Posterior relationships matter in retrocecal appendicitis, where inflammation can track toward the psoas and posterior parietal peritoneum and pain patterns shift.
